TUITION AND POLICY AGREEMENT

FINANCIAL OBLIGATIONS:

* Enrollment is confirmed after receipt of completed application and accompanying $50 registration fee

* Once the application is submitted and processed, you will receive a statement requesting a $500 deposit to hold your child’s spot.

LATE FEE:

* Dismissal times: Half Day 12:15 p.m. , Full Day 3:15 p.m. , Late Care 4:30 p.m. or 5:00 p.m.

* Each family will be billed a yearly security fee of $200.

PHOTO RELEASE:

I give permission for my child's picture to be used on the Old York Road Temple-Beth Am website and in any advertisements, and publications.I DO NOT give permission for my child's picture to be used on the OYRTBA website and in any advertisements, and publications. However, my child may be in pictures sent via email or private class Facebook page on school bulletin board.

UPDATING STUDENT INFORMATION:
